Act 34 (PA Criminal History)     Website

  • PA State Police website at https://epatch.state.pa.us Response is immediate.
  • Reason for Clearance: Employment
  • Payment of $8.00 (credit card)
  • Please be sure to print the actual credential by clicking on the control number issued and then certification.

Act 114 (FBI Fingerprinting)      Website and Physical Location

  • Register online with Payment of $27.00 (credit card) at pa.cogentid.com. Once on the main site, you must click on the Pennsylvania Department of Education (PDE) block.
  • Reason for Clearance: Area Vocational-Technical School
  • Proceed to fingerprint site of choice. Location of fingerprint sites and hours of operation for each site are posted on Cogent Systems’ website. (Bloomsburg University is now a fingerprinting site)

Act 151 (Child Abuse)     Website

  • You may apply online at: https://www.compass.state.pa.us/CWIS
  • Create an account
  • Reason for clearance application: School Employee Governed by PA School Code
  • Payment of $8.00 (credit card)
  • Once your application has been processed, you will receive a notification via email to log in to your account and view the outcome/result of the application and print certification.

Act 168 (PA Sexual Misconduct/Abuse Disclosure Release Form)

By law as of 12/22/14, one form must be completed for each of the following:

  • Current Employer
  • Each former employer that was a school entity
  • Each former employer where you were employed in a position that involved direct contact with children
  • Applicant completes all of page 1 – must give complete employer information – be sure to sign and date the top of page 2.
  • Instructions page for correctly completing the questions in Section #1are found on page 3.
  • Disclosure Release form (s) must be submitted at the time of application.

 The law now requires all school employees, contractors and volunteers having direct contact with children to obtain new clearances (child abuse background checks, state and federal criminal history checks) every 60 months.
